Data is divided in packets, and these packets travels independently to each other.

Packet destined for the system on the same local network

Packet destined for a remote system on a different network.

When the packet is reaches to the router, then router will look into the destination address that where this packet needs to send. If is understand by the router's forwarding packet will send it to the appropriate place. If the router is not able to identify the destination, then it will farward the packets to it's defined gateway, this process will repeat again and again.
