Assessment is the feedback from the student to the instructor about the student's learning while evaluation is the feedback from the instructor to the student about the student's learning.
Assessment is the process of recording and interpreting various data to come up with a proper measurement of skills, knowledge and attitude to improve the overall learning experience while evaluation is the process of judging someone based on his significance, merit and knowledge using a set of given rules and methods.
The results from assessment are always constructive feedback while the results from evaluation mainly highlight shortcomings.
Assessment is graded while evaluation is not graded.
