Only HCOOH solution and solution of HCOOH with NaCl gives equivalent ionization. In HCOOH and NaOH solution there is a acid base reaction so the ionization rate will be higher.

In HCOOH with HCl solution there is a common ion effect of H+ ion. Thats why in the sollution "d" there will be lowest rate of ionization of HCOOH.
