Why a lens form an image? If the answer is glass, why a glass slab doesn't form an image

A flat glass plate cannot form an image because it is flat and all incident light refract at the same angle. The surface of the lens, on the other hand, is curved, which means that the angle of refraction is constantly changing, making it possible to form images.
