Jack need 15 cm3 of 40% and 10 cm3 of 28% mixture.

Let "x" represent amount of first mixture and "y" represent the amount of second mixture.


Then the first equation (eq1) states, that total volume is 25 cm3.

There are 0.4 x grams of the nutrient in x cm3 of 40% mixture.

Accordingly, there are 0.1 y and 0.28×25 grams of the nutrient in y and 25 cm3.

So the second equation is based on that fact. The total solution in Mathematica is shown below:
