Medical errors are categorized into different categories depending on how they occur to patients. They can be divided into diagnostic, treatment, preventive and others. Diagnostic errors are caused by miscalculation or delay in diagnosis, the use of old-fashioned test or therapy and failure to employ indicated test. Treatment errors occur due to errors in performance of operation and other procedures. There is also error in inappropriate care and avoidable delays in the treatment.

           Preventive errors occur due to inadequate monitoring or follow-up of treatment, and failure to provide prophylactic treatment. Other minor errors are based on the failure of communication between the medical practitioners, equipment failures and other related system failures.
