High density lipoprotein (HDL) mediates the transport of cholesterol from peripheral tissues to the liver in a process called reverse cholesterol transport. HDL-derived cholesterol is then de-esterified and secreted into the bile. This can occur either as free cholesterol or as bile acids. Secretion of bile salts and cholesterol into the bile by the liver is the only mechanism by which cholesterol is excreted.

In the reverse transport cholesterol returned back to the liver via the plasma compartment. Two thirds of the hepatic blood flow is unoxygenated blood rich in the products of digestion, which is delivered by the portal vein
